I go to one of those schools that uses the Peds CLIPP exam as a major part of the clerkship and final grade instead of a shelf. Most people say 'just read the case summaries/know the cases cold', but going through these cases I cant help but feel like there are important things missing/ (lots of fluff with random minutae in those cases). The biggest thing is theres no real practice questions for this thing, which is how I usually prep when I have shelf's so just trying to get some advice/ idea of what to expect of this thing (difficulty, etc). TLDR: Peds clipp case exam determines our grade, dont really know how to study for it b/c cases sux UPDATE: Took the thing. Barely qualified for H ( 88%).
I will say this, knew the summaries COLD and still felt it was hard. Yes 50% is perhaps verbatim there, but I'd say the other half felt very much like a shelf. Uworld and random things picked up from other rotations seemd to help. Our school tried the fmcases shelf instead of the NBME one for family medicine. I thought it was horriblely written and didn't reflect what you need to know for preventative medicine. If it's anything like that then you got to memorize like all the algorithms, especially the ones with scoring in them. They love to test stupid minutia like that.
I had more than one question on the fmcase that was like 's/sx of strep throat, what to do next' and then the answer choices are 'amox or rapid strep test'. It turned out to be amox because with enough points on this stupid algorithm, you technically don't need to rapid strep the guy. It doesn't reflect clinical practice at all. Or the algorithm/scoring system for acid reflex, or xrays, etc etc. It's a lot of clinical algorithms. Sure, it's taken from the cases, but the cases take forever to go through and are full of stupid, unnecessary information. Harrison's Internal Medicine is higher yield than those cases.
At least the NBMEs are straightforward. Those tests rarely trick you like that.

You have to know your s/sx and what to do with them, but very few questions will be like the one above. For the record, I scored a 69 on it which barely qualified me for honors. By comparison I've been scoring in the 80-90th percentile for all the other NBMEs taken so far.
